What is a Form 115B?
A Form 115B is a medical declaration used by British Skydiving. Some individuals are required to obtain medical clearance from a doctor before taking part in skydiving activities.
You may need a Form 115B if:

You have certain medical conditions. -
You take regular prescription medication. -
You have previously been advised that medical clearance is required. -
Your skydiving provider has asked you to obtain a doctor’s signature before your jump.
The purpose of the assessment is to ensure that there are no medical conditions that would make skydiving unsafe.
What Happens During the Appointment?
During your appointment, one of our doctors will:

Review your medical history. -
Discuss any current medical conditions. -
Review any medications you take.

Review relevant medical records where required. -
Assess whether you meet the medical standards for completion of the form. -
Complete and sign the Form 115B if appropriate.
Most appointments take approximately 15–20 minutes.
