A Combined Blood Test and Ultrasound Assessment
Ovarian reserve cannot be fully assessed through one isolated test.
An AMH blood test measures a hormone produced by small ovarian follicles. An antral follicle count, or AFC, uses ultrasound to count the small follicles visible within the ovaries.
Our ovarian reserve assessment combines both measurements with a pelvic fertility ultrasound and doctor-led interpretation.
This provides a broader and more clinically useful assessment than an isolated AMH result or ultrasound scan alone.
What Is Ovarian Reserve?
Ovarian reserve refers to the approximate number of eggs remaining within the ovaries.
Women are born with their lifetime supply of eggs. The number gradually declines with age, although the rate of decline differs between individuals.
Ovarian reserve testing can provide information about egg quantity and may help estimate how the ovaries could respond to stimulation during fertility treatment.
However, ovarian reserve testing does not measure egg quality and cannot predict with certainty whether someone will conceive naturally.

Why Combine AMH and AFC?
AMH and AFC assess ovarian reserve in different but complementary ways.

AMH measures a hormone produced by small ovarian follicles.

AFC counts the follicles that can be seen during ultrasound.

Pelvic ultrasound also assesses the structure of the uterus and ovaries.

Medical review places both results in the context of your age, history and fertility plans.
Occasionally, AMH and AFC results do not appear to correspond. Reviewing them together allows a doctor to consider possible reasons and decide whether any further assessment is appropriate.
Neither measurement should be interpreted as a simple fertility score.

An ovarian reserve assessment provides useful information about egg quantity, but it is not a complete measure of fertility.
It cannot:

Guarantee a future pregnancy

Determine the exact number of eggs remaining

Predict precisely when menopause will occur

Confirm whether the fallopian tubes are open

Assess sperm quality

Identify every possible cause of difficulty conceiving

Replace a full specialist fertility assessment when one is required
Your doctor will explain these limitations before interpreting your results.
Is the Ultrasound Transvaginal?
An antral follicle count is usually most accurately performed using a transvaginal ultrasound.
A slim ultrasound probe is gently inserted into the vagina, allowing detailed images of the ovaries, follicles, uterus and endometrial lining.
The procedure will be explained beforehand and will only proceed with your consent. Most patients tolerate the examination well, although mild discomfort is possible.
You can ask for the examination to stop at any time. A chaperone can be provided, and you may bring a partner, friend or relative for support.
If you do not wish to have a transvaginal examination, please tell us. A transabdominal scan may sometimes be possible, although it may not provide sufficient detail for an accurate antral follicle count.
When Should I Have the Assessment?
AMH can usually be measured at any point during the menstrual cycle.
An antral follicle count is commonly performed during the early part of the cycle, often between days 2 and 5. This provides a baseline assessment before a dominant follicle has developed.

How Quickly Will I Receive My Results?
Your ultrasound findings will normally be explained during the appointment.
The AMH result is usually available within a few working days. Once it is received, a doctor will review the result alongside your antral follicle count, ultrasound findings and clinical history.
You will then receive a personalised explanation and recommendations where appropriate.

What Happens If My Ovarian Reserve Appears High?
A higher AMH level or antral follicle count can reflect a greater number of ovarian follicles.
Higher results can also be associated with polycystic ovarian morphology or polycystic ovary syndrome. However, neither AMH nor ultrasound findings alone can diagnose PCOS.
Your doctor will consider your menstrual cycle, symptoms, hormone results and ultrasound findings before recommending any further assessment.
